Adventure Quest Field Trip & connections event ( all ages)

This will include 2 pieces of pizza per child and unlimited drinks. It will also include laser tag, golf, bumper cars and  arcade. 

By filling out this form, I understand that as a parent that I am responsible and liable for my child(ren) as well as the well being and release S.E.R.V.E. Homeschool Outreach (including leadership, chaperones)  for all liability and responsibility on all outreach and events and field trips.  I also understand that I will not be able to receive a refund after the payment due date of Friday, September 13th.
